Rapid Response Case Management
The Rapid Response Case Management (RRCM) pilot, is a strategic partnership between DayBreak, Alameda County Emergency Medical Services (EMS), hospitals, fire departments and Adult Protective Services, to address the issue of high utilization of emergency services by elders for non-emergency causes. Through a very intensive case management, we increase their access to housing and community based health and social services, improve their quality of lives and reduce their health care costs.
Please note DayBreak is accepting referrals to this program only from designated partner agencies.
