
Centers provide nursing care, chronic disease management, dietitian and rehabilitative services, social services, and stimulating cognitive and recreational activities. We serve culturally diverse, elderly adults at risk of institutional care with the goal of restoring, maintaining or improving participants’ overall functioning, quality of life, and independence to remain living at home longer.
