DayBreak’s Rapid Response Case Manager/Community Liaison Position 100% FTE

The Rapid Response Case Manager (RRCM)/ Community Liaison of DayBreak holds a key role in a small yet very dynamic organization with a critical mission. The RRCM will be part of a leading team that designs and implement a new pilot, working on connecting older adults who use emergency medical and ED services frequently to community resources.
The RRCM is a social worker who knows health and social services in the community, and works closely with clients to help them regain or maintain their health and well being. The RRCM will meet the elder who called 911 multiple times at the hospital, as close as possible to the time they were transferred, conduct an initial assessment, and will walk the elder through every step of getting the resources they need, taking in consideration needs, wishes, existing support systems and other factors that can determine better health outcomes.
As DayBreak’s Community Liaison, you will support our Emergency Food Bags Delivery Operation, as well as other tasks to support the case management team with their clients in the community.
As a member of a small team, the RRCM will also take part in other activities we will have, such as the Annual Event, donor’s campaign, and other tasks in the office as needed.

Work closely with hospital and EMS teams to provide a timely case management to older adults who were identified as candidates to the RRCM services.
Work with the pilot team to identify, develop and asses the tools and protocols for the RRCM tool kit.
Coordinate care plan with multiple partners and provide ongoing coordination of support services for clients and their families.
Perform client assessments, develop treatment plans and participate in meetings and case reviews as required.
Arrange all services as appropriate monitor their effectiveness and reassessment of client needs, including follow up on the care plan execution.
Intervene in acute situations when community services are not immediately available, include the provision of transportation, emergency food or other appropriate services.
Maintain program statistics and ensure the completion of all required reports in a timely manner.
Support the delivery of food bags to seniors in the community, as well as other tasks related to the Emergency Food Bags Delivery Operation as needed.
Support the case management team with other tasks related to their clients in the community as needed.
Perform other duties as assigned.
Bachelor’s degree or equivalent required, an MSW desired.
Experience in social welfare, gerontology, human development or related discipline.
Familiarity with hospitals’ work environment and ability to work closely with hospitals’ teams.
Excellent background and knowledge of issues related to Geriatrics.
Ability to interact effectively with both clients, caregivers and professionals of all backgrounds.
Excellent interviewing and communication skills.
Working knowledge of Alameda County resources, and experience working with social service agencies and senior services desirable.
Ability to work in a highly collaborative environment, and be part of a small office team.
Ability to work independently, think ‘out of the box’ and be self motivated.
Ability to prioritize and manage multiple tasks, and to perform under conditions requiring speed and accuracy.
Experience in Program Development desirable.
Excellent speaking, writing, and organization skills.
Computer literate. DayBreak has both PCs and Apple computers and utilizes Microsoft Office software and Salesforce data system.
Flexible hours to accommodate occasional night or weekend assignments.
Valid California driver's license, automobile insurance and access to an automobile.
Sitting : 45 to 1 hour while driving within the community.
Stooping intermittent 5 to 10 minutes
Standing intermittent standing within the 8 hour shift
Reaching intermittent within a 8 hour period
Typing intermittently within a 8 hour period

DayBreak’s office is located in DT Oakland. However, this position requires traveling to different locations in Alameda County.
Hours and Compensation
This is full time, 100% FTE position. Salary is based on experience, and is competitive with small sized non-profits.
Benefits include health and dental insurance, nine paid holidays, two personal days, accrued sick leave and vacation time.
