The Rapid Response Case Management Manager (RRCM) of DayBreak holds a key role in a small yet very dynamic organization with a critical mission. The RRCM Program Manager will take part of the designing and implementation of a new pilot, working with older adults who use emergency systems frequently to connect them to community resources and reduce their emergency systems’ utilization.
The RRCM Program Manager is a social worker or possess a similar background, who knows health and social services in the community, and works closely with the Case Management team to help clients regain or maintain their health and well being. The RRCM Program Manager will supervise the work of the Case Management team, as well as provide direct services to clients when appropriate; meet the elder who called 911 multiple times at the hospital, as close as possible to the time they were transferred, conduct an initial assessment, and will walk the elder through every step of getting the resources they need, taking in consideration needs, wishes, existing support systems and other factors that can determine better health outcomes.
As a member of a small team, the RRCM Program Manager will also take part in other activities we will have, such as the Annual Event, donor’s campaign, and other tasks in the office as needed.

Work closely with DayBreak’s partners, including hospitals and EMS, Fire Departments and Adult Protective Services teams to streamline the process of referrals and ensure that a timely case management is provided to older adults who were identified as candidates to the RRCM services.
Provide Supervision to the Case Management team, including individual supervision and leading a weekly team meetings.
Recruit and retain case managers, and ensure appropriate on-boarding and on-going training is provided to the case management team.
Work with partner organizations in the community to maintain and expand collaboration between the different agencies and DayBreak.
Participate in external and internal meetings related to the RRCM Program as required.
Work with DayBreak’s leadership team, including the Director of Care Coordination, the Executive Director and the Housing Program Manager to identify, develop and asses the tools and protocols for the RRCM tool kit.
Support the case managers in coordinating care plan with multiple partners and provide ongoing coordination of support services for clients and their families.
Perform client assessments, develop treatment plans and participate in meetings and case reviews as required.
Arrange all services as appropriate monitor their effectiveness and reassessment of client needs, including follow up on the care plan execution.
Intervene in acute situations when community services are not immediately available, include the provision of transportation, emergency food or other appropriate services.
Maintain program statistics and ensure the completion of all required reports in a timely manner.
Perform other duties as assigned.
Bachelor’s degree or equivalent required, an MSW desired.
Two years of experience in program management and staff supervision.
Experience in social welfare, gerontology, human development or related discipline.
Familiarity with hospitals’ work environment and ability to work closely with hospitals’ teams.
Excellent background and knowledge of issues related to Geriatrics.
Ability to interact effectively with both team members, clients, caregivers and professionals of all backgrounds.
Excellent interviewing and communication skills.
Working knowledge of Alameda County resources, and experience working with social service agencies and senior services desirable.
Ability to work in a highly collaborative environment, and be part of a small office team.
Ability to work independently, think ‘out of the box’ and be self motivated.
Ability to prioritize and manage multiple tasks, and to perform under conditions requiring speed and accuracy.
Excellent speaking, writing, and organization skills.
Computer literate. DayBreak has both PCs and Apple computers and utilizes Microsoft Office software and ExtendedReach data system.
Flexible hours to accommodate occasional night or weekend assignments.
Valid California driver's license, automobile insurance and access to an automobile.
DayBreak’s office is located in DT San Leandro. However, this position requires traveling to different locations in Alameda County.
Hours and Compensation
This is full time, 100% FTE position. Salary is based on experience, and is competitive with small sized non-profits.
Benefits include health and dental insurance, nine paid holidays, two personal days, accrued sick leave and vacation time.
