
Center for Elders’ Independence (CEI) PACE programs provide full 24/7 medical care along with adult day health care. Participants also get transportation to and from medical appointments and receive home care when necessary. A team of healthcare professionals coordinate each senior’s care so frail seniors can stay living at home. If necessary CEI also pays for hospitalizations and nursing home rehabilitation for no additional co-pay. Participants must be 55 or older, live in CEI’s service area between Hayward and El Sobrante, and be eligible for nursing home care.
